Responsibilities
- Deliver direct nursing care to residents within scope of practice, with accurate documentation and medication administration as allowed by policy
- Monitor residents’ conditions, record vital signs, report changes to the supervising RN, and respond to clinical concerns promptly
- Assist with activities of daily living and mobility, providing comfort measures and fostering resident dignity
- Perform routine procedures (e.g., wound care, specimen collection) according to facility protocol and care plans
- Collaborate with the interdisciplinary team to implement and update care plans; contribute to quality and safety initiatives
- Maintain infection control standards, safety procedures, and accurate charting in accordance with regulations
Requirements
- Current LPN license for New Hampshire (active and in good standing)
- Prior experience in a skilled nursing facility or long-term care setting preferred
- Ability to perform essential nursing duties and adhere to facility policies
- Proficiency with basic charting/electronic medical records and clear communication
- Availability for an 11-week assignment and the listed shift (7:00–15:30), 5 days per week
