Responsibilities
- Plan, organize, and direct home care services to meet patient needs in the home setting.
- Manage a patient caseload in an effective and efficient manner.
- Provide patient care independently while collaborating with other core disciplines to meet care goals.
- Function as a resource to patients, family members, and other members of the care team.
- Document care plans, monitor patient progress, and adjust plans in accordance with the patient’s plan of care.
Requirements
- Active Registered Nurse license in Oregon (or eligibility to practice in Oregon)
- Ability to provide patient care in a home health setting and manage a caseload
- Strong communication skills and ability to collaborate with patients, families, and interdisciplinary teams
- Demonstrated ability to work autonomously with appropriate clinical supervision as part of a home-based care team
- Willingness to participate in a 13-week travel assignment beginning 04/20/2026
- Day shift availability (8-hour shifts)
