Responsibilities
- Conduct comprehensive speech, language, voice, and swallowing assessments for adult residents in the LTC setting.
- Develop, implement, and monitor individualized treatment plans aligned with resident goals and interdisciplinary care plans.
- Provide direct therapy to improve communication, cognitive-communication, and swallowing function; document progress in the medical record.
- Perform screenings and referrals as appropriate; participate in discharge planning and care conferences with the team.
- Collaborate with physicians, nurses, occupational and physical therapists, and social services to optimize patient outcomes.
- Ensure regulatory and facility policy compliance; maintain accurate documentation and appropriate coding.
Requirements
- Current Speech-Language Pathologist license/registration in the United States; specifics to be confirmed during onboarding.
- Ability to work in a long-term care setting for a 13-week travel assignment; Days shift availability.
- Strong assessment, treatment planning, and documentation skills; experience with adult populations.
- Excellent communication and collaboration within an interdisciplinary team.
- Proficiency with electronic health records and standard medical documentation.
- Willingness to travel to and work in Glendive, MT for the duration; 36 hours per week guaranteed.
