Licensed Practical Nurse – Long-Term Care in Rochester, NY. This is a travel, temporary assignment starting 07/06/2026 for 13 weeks, with shifts of 12 hours (3x12s). Responsibilities
- Deliver direct patient care to geriatric residents in a long-term care setting, including medication administration and documentation
- Monitor residents, document changes, and communicate with the interdisciplinary team
- Oversee nursing assistants on the unit (approximately 4–6 CNAs per LPN) to ensure care consistency
- Provide wound care as prescribed and participate in care planning and routine assessments
- Maintain patient safety, dignity, and independence, while adhering to infection control and regulatory requirements
- Demonstrate independent critical thinking and professional judgment in daily nursing tasks
Requirements
- Active New York LPN license
- BLS certification from the American Heart Association
- Minimum of 2 years of long-term care experience
- Experience with wound care and medication administration in a geriatric LTC setting
- Ability to work 12-hour shifts and function effectively in a rotating D/E/N schedule
- Strong communication skills and ability to work independently within a multidisciplinary team
