Responsibilities
- Provide skilled nursing care to hospice patients in home settings, including symptom assessment, comfort measures, and end-of-life care planning.
- Administer prescribed medications, monitor patient responses, and manage therapies as appropriate.
- Collaborate with the hospice interdisciplinary team to coordinate care, communicate changes, and support families.
- Document care activities in the electronic medical record, maintain accurate assessments, and ensure regulatory compliance.
- Educate patients and family caregivers on care plans, medication usage, symptom management, and safety at home.
Requirements
- Active California RN license
- BLS/CPR certification
- Minimum 1 year of RN experience; hospice or home health experience preferred
- Ability to work a night shift schedule (3x12s) and travel as required
- Strong communication and patient-family education skills
