Responsibilities
- Provide direct patient care to long-term care residents under supervision, including medication administration and routine clinical tasks per physician orders.
- Monitor residents’ conditions, document vital signs, pain levels, and changes; communicate findings promptly to the supervising RN.
- Assist with activities of daily living, mobility, and safety to support resident independence and comfort.
- Maintain accurate nursing documentation and care plans; ensure compliance with LTC regulations and facility policies.
- Collaborate with the care team, participate in rounds, and implement care plans efficiently; support an environment that minimizes orientation requirements.
- Adhere to infection control practices and standard precautions; respond to calls and emergencies in a timely manner.
Requirements
- Active LPN license in good standing (state-specific)
- Must have LTC experience and PCC experience
- Ability to work the night shift: 19:00–07:30
- Prior experience providing direct patient care in a long-term care setting
- Strong documentation, communication, and teamwork skills
- Diploma from an accredited practical nursing program (or equivalent) and ability to meet applicable licensure requirements
