Travel Physical Therapist Job
Physical Therapist – Travel assignment in Grand Junction, Colorado. Start date 05/11/2026 for 13 weeks. Weekly pay range: $2,021–$2,119 per week. Guaranteed hours: 0. Shift: 5x8 Days. Setting: Urgent Care Center – Outpatient. This travel assignment supports the Program of All-inclusive Care for the Elderly (PACE), delivering coordinated, whole-person PT services in collaboration with an interdisciplinary team.

Responsibilities
- Perform initial assessments for potential PACE participants and annual reassessments; document findings and contribute to plan of care.
- Develop, update, and implement an individualized plan of care (POC) in collaboration with participants, families, and the interdisciplinary team to improve mobility and daily functioning.
- Deliver PT interventions to enhance safety and independence with activities of daily living, and monitor changes in condition to adjust the POC as needed.
- Provide clinical education and training to PACE staff and volunteers to maximize safety and functional abilities of participants.
- Maintain accurate records in the participant’s chart, communicate with the team, and coordinate care with other providers to ensure comprehensive service delivery.
- Support participants in diverse environments, including home-based settings, and promote best practices for aging in place.
Requirements
- Education: Graduate from a CAPTE-accredited Physical Therapy program.
- Licensure: Current Colorado Physical Therapist license.
- Experience: Minimum 1 year of PT experience; 2–3 years of home health or other home-based PT experience preferred.
- Certifications/Other: Current CPR certification; valid driver’s license with reliable transportation and adequate automobile insurance.
- Skills: Ability to work effectively in a variety of environments; excellent interpersonal and written/verbal communication skills.
