Responsibilities
- Provide direct nursing care for med-surg patients within a home healthcare - hospice setting, including assessment, monitoring, and symptom management.
- Administer medications and treatments per physician orders, utilizing the Pyxis system; manage IV therapy as applicable.
- Document in the Cerner EMR, perform bedside shift reports, and communicate clearly with the care team using AIDET principles.
- Develop and adjust patient care plans, coordinate admissions or transfers, and collaborate with physicians and other clinicians to optimize patient outcomes.
- Educate patients and families on care plans, medication management, safety, and disease processes; address questions and provide supportive counseling.
- Ensure infection control, patient safety, and quality of life considerations; adapt to tele-staffing ratios and occasional facility-level responsibilities as needed.
Requirements
- Active Registered Nurse license in the state of California
- BLS/CPR certification required
- Ability to work in a home healthcare - hospice setting; comfortable with 12-hour day shifts and rotating weekends
- Proficiency with Cerner EMR and Pyxis medication system
- Strong clinical assessment, documentation, and patient-family communication skills
- Experience in medical-surgical or hospice/home health settings preferred
