Responsibilities
- Conduct psychosocial assessments for behavioral health patients in an acute care setting.
- Develop and implement discharge planning, coordinating with the multidisciplinary team to ensure appropriate community and inpatient support.
- Provide psychosocial counseling to patients and families, including crisis intervention as needed.
- Document assessments, care plans, and progress notes in the medical record adhering to policy and regulatory requirements.
- Facilitate referrals to community resources, social services, and follow-up care to support safe transitions.
- Collaborate with team members to optimize patient outcomes and coordinate care across services.
Requirements
- Master’s Degree in Social Work (MSW); CA and HI requirements as specified in job notes.
- One year of experience in an acute or long-term medical setting; two years’ experience preferred.
- Must have worked in the submitted unit specialty within the last 3 months.
- Ability to work in a fast-paced acute care environment and engage with diverse patients and families.
- Current credentialing and eligibility to practice social work per applicable state regulations (as noted).
