Responsibilities
- Provide psychosocial assessments and develop care plans for patients within Behavioral Health in an acute care setting.
- Lead discharge planning and coordinate with the multidisciplinary team to ensure safe transitions of care.
- Deliver patient and family education, identify psychosocial barriers, and connect patients with community resources and support services.
- Document outcomes, assessments, and care plans accurately in the electronic health record; maintain compliance with regulatory and facility policies.
- Participate in care conferences, crisis intervention when needed, and advocacy efforts to support patient rights and safety.
Requirements
- Education: Master’s Degree in Social Work; in California (CA) and Hawaii (HI), MSW is required.
- Experience: Minimum 1 year of acute or long-term medical setting experience; 2 years’ experience in an acute or long-term medical setting is preferred; must have worked in the submitted unit specialty within the last 3 months.
- Licensure/Certification: Licensure requirements to be confirmed per Job Notes; verify during submission.
- Skills: Ability to work effectively in a fast-paced acute care environment; strong collaboration with a multidisciplinary team; ability to manage complex psychosocial needs.
- Eligibility: Must be able to work in the listed setting and follow unit-specific policies and procedures.
