Responsibilities
- Conduct comprehensive OT evaluations for adult patients and develop individualized treatment plans to enhance activities of daily living, mobility, and functional independence.
- Implement evidence-based therapy interventions appropriate for the SNF setting with attention to acute care considerations.
- Collaborate with an interdisciplinary team to support discharge planning, care coordination, and safety with prescribed therapies.
- Document patient progress in the electronic health record, ensuring timely, accurate notes and compliant recordkeeping.
- Educate patients and families on therapy goals, progress, and home exercise programs; provide training for safe activity performance.
- Monitor patient response to therapy, adjust treatment plans as needed, and address any clinical concerns with the team.
Requirements
- Active occupational therapy license in good standing (state licensure to be verified).
- Master's or Doctoral degree in Occupational Therapy from an accredited program.
- Minimum 1 year of OT experience in adult rehabilitation, acute care, or skilled nursing settings preferred.
- Current Basic Life Support (BLS) certification.
- Ability to work in a travel assignment with 35 weekly hours and the listed shift in a skilled nursing facility environment.
- US work authorization.
