Responsibilities
- Provide direct patient care to residents within the LPN scope of practice in a long-term care setting
- Monitor, assess, and document patient status, vital signs, and responses to treatments
- Administer medications and treatments as prescribed, following facility policy and procedure
- Collaborate with RNs and interdisciplinary team to develop and update individualized care plans
- Assist residents with activities of daily living and implement safety and infection control measures
- Recognize changes in condition and promptly report to the supervising nurse for ongoing care decisions
Requirements
- Active Licensed Practical Nurse license in Alabama or eligibility for licensure
- Ability to work 3x12-night shifts and maintain reliable scheduling in a long-term care environment
- Strong communication and documentation skills
- Comfortable functioning in a fast-paced skilled nursing setting
