Overview

Responsibilities
- Provide direct and indirect skilled nursing care for residents on the assigned unit under the supervision of a Registered Nurse.
- Collect data on residents’ physical and psychosocial conditions and communicate findings to the RN and Interdisciplinary Team.
- Contribute to development of an age-appropriate, culturally relevant plan of resident care reflecting nursing diagnoses and interventions; involve residents and families in the process.
- Recommend revisions to the plan of care after evaluating resident outcomes and refer to appropriate services to address all aspects of care.
- Maintain current knowledge and competency in Nursing Practice; participate in continuing education and in-service programs.
- Demonstrate leadership in the professional practice setting and collaborate with other nursing staff, physicians, and ancillary team members; provide mentoring to peers as needed.

Requirements
- Active NY LPN licensure
- BLS certification (through American Heart Association or RQI)
- Minimum 1 year of LPN experience, preferably in a long-term care setting
- Ability to work night shifts and weekends/holidays as scheduled; adapt to the 5x8 night rotation
- Competence in caring for adult and geriatric residents; strong collaboration with an Interdisciplinary Team
- Willingness to follow defined policies and procedures under RN supervision and maintain patient safety
