Responsibilities
- Deliver coordinated nursing care for a patient or assigned group of patients according to established standards of care and the nursing process.
- Assess patient status, administer prescribed medications, monitor vital signs, and respond to changes in clinical condition.
- Develop and update care plans in collaboration with the interdisciplinary team; document care accurately and promptly.
- Supervise and direct activities of various levels of assigned nursing staff; coordinate care with other disciplines to ensure safe and effective delivery of services.
- Utilize critical thinking, professional judgment, and independent decision-making within the scope of practice; ensure adherence to policies and quality standards.
- Maintain patient safety and comfort in the home hospice setting, including infection control practices and timely escalation when needed.
Requirements
- Active, unrestricted RN license (state-specific) with eligibility to practice in the assignment location.
- Ability to work 12-hour day shifts across a three-day-per-week pattern.
- Comfort delivering nursing care in a home healthcare – hospice environment and coordinating with an interdisciplinary team.
- Strong clinical assessment, critical thinking, and supervisory capabilities.
- Demonstrated ability to document care clearly and maintain compliant records (electronic systems as applicable).
