Responsibilities
- Provide direct resident care in a long-term care setting under supervision of a registered nurse or supervisor
- Administer medications and document accurately in the charting system
- Monitor patient status, vitals, and changes in condition; communicate promptly to the nursing team
- Assist residents with activities of daily living and mobility, promoting dignity and safety
- Maintain infection control standards, safety procedures, and a clean care environment
- Collaborate with an interdisciplinary team to implement and adjust individualized care plans
Requirements
- Active LPN license in good standing in the state of New Hampshire
- Experience in long-term care or skilled nursing settings preferred
- Ability to work the overnight shift and meet the guaranteed 40 hours per week
- Strong communication and documentation skills
- Comfortable performing required duties in a long-term care setting
