Responsibilities
- Conduct comprehensive speech-language pathology assessments, develop treatment plans, and provide skilled therapy to residents in a long-term care setting.
- Implement evidence-based therapy approaches; monitor progress, adjust goals, and document outcomes to support residents’ communication and swallowing needs.
- Perform bedside or in-room evaluations; determine prognosis and guide individuals toward maximum functional potential.
- Collaborate with the interdisciplinary care team; communicate with residents, families, and staff; participate in care planning and discharge planning as appropriate.
- Maintain accurate documentation in the electronic medical record (Epic/EMR) and align with PDPM reimbursement requirements; complete progress notes and program updates.
Requirements
- Active New York State license as a Speech-Language Pathologist
- Eligible for or holding the Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA
- Degree in Speech-Language Pathology from an ASHA-accredited program
- LTC experience required
- Proficiency with MS Office and Epic EMR software
- Knowledge of PDPM reimbursement system
- Excellent communication skills for working with patients, medical staff, and care teams
- Ability to work in a long-term care setting
