Responsibilities
- Administer prescribed medications and document administration accurately in the chart
- Monitor resident status, perform routine assessments, and report changes to the supervising RN
- Provide direct patient care, including assistance with activities of daily living and routine nursing tasks
- Assist with wound care, vital signs, and routine procedures as delegated
- Complete timely and accurate documentation in the electronic health record; support care planning and updates
- Maintain safety, infection control, and falls prevention standards; collaborate with the interdisciplinary team
Requirements
- Active LPN license in Alabama (or eligibility to practice as an LPN in Alabama)
- Minimum 1 year of experience in a long-term care or skilled nursing setting
- Ability to work 3x12-hour night shifts (7:00 p.m. – 7:30 a.m.)
- Strong communication and teamwork skills; ability to document clearly and work with an interdisciplinary team
