Responsibilities
- Administer prescribed medications and therapies under supervision of an RN
- Monitor and document patient vital signs, symptoms, and response to treatment
- Perform basic wound care, dressing changes, and assist with routine nursing procedures
- Support activities of daily living and mobility per care plans
- Maintain accurate charting and medication administration records; communicate changes to the RN
- Collaborate with the interdisciplinary team to ensure safe, quality care and infection control
Requirements
- Active Licensed Practical Nurse (LPN) license
- Completion of an approved LPN program; current in good standing
- Minimum 1 year of experience in long-term care or skilled nursing settings preferred
- Ability to work 3x12-hour night shifts; travel assignment readiness
- Current CPR/BLS certification
- Strong communication and documentation skills; ability to follow care plans
