Responsibilities
- Conduct comprehensive PT evaluations for residents in a geriatric long-term care setting and establish individualized treatment plans.
- Implement therapeutic interventions to improve mobility, balance, gait, strength, and activities of daily living.
- Deliver direct patient therapy sessions, track progress, and adjust plans of care as needed.
- Document assessment findings, treatment sessions, and outcomes in the facility’s electronic health record per policy.
- Collaborate with physicians, nursing staff, and therapy colleagues to coordinate care and support discharge planning and transitions.
- Ensure adherence to safety, regulatory, and documentation standards while maintaining ethical and professional practice.
Requirements
- Active Physical Therapist license in good standing; New Hampshire licensure or eligibility for NH licensure
- Graduation from an accredited physical therapy program (CAPTE) and eligibility for licensure
- Ability to work in a long-term care/geriatric rehabilitation environment and engage with residents respectfully
- Strong documentation skills and familiarity with standard PT assessments and modalities
- Availability to work the 35-hour-per-week schedule and participate in a travel assignment
