Responsibilities
- Provide direct patient care to residents in a long-term care setting under RN supervision
- Administer medications and treatments per physician orders and facility policy
- Monitor residents’ vital signs, changes in condition, and report findings promptly
- Assist with activities of daily living and mobility to support resident independence
- Document care plans, progress notes, and any changes in condition accurately
- Collaborate with the interdisciplinary team to implement and update individualized care plans
Requirements
- Active LPN license in New Mexico or eligibility to practice in NM
- Completion of an approved LPN educational program (LPN diploma)
- Prior long-term care or geriatric nursing experience preferred
- Ability to work 12-hour shifts (06:00–18:30) in a long-term care setting
- CPR/BLS certification required
- Strong assessment, documentation, and teamwork skills
