Responsibilities
- Perform comprehensive physical therapy evaluations and develop individualized plans of care for residents in a SNF setting.
- Deliver clinical interventions including therapeutic exercises, gait training, balance and neuromuscular re-education as prescribed.
- Implement, monitor, and adjust treatment plans based on resident progress; document progress notes in line with state and facility requirements.
- Provide patient and family education on exercise programs, safety, functional goals, and discharge planning.
- Collaborate with physicians, nursing staff, and other therapy team members to optimize resident outcomes.
- Ensure safe patient handling, promote fall prevention, and uphold infection control and standard precautions during therapy sessions.
Requirements
- Active New Mexico physical therapy license (PT) or physical therapy assistant (PTA) licensure.
- Eligibility to work in the United States and travel to the assignment location.
- Ability to work in a skilled nursing/long-term care setting.
- Availability to meet the guaranteed 35 hours per week for the 13-week assignment starting 10/12/2026.
- Strong communication and clinical skills to coordinate with an interdisciplinary care team.
