Responsibilities
- Deliver physical therapy evaluation, treatment planning, and ongoing rehabilitation for residents per the plan of care.
- Implement evidence-based interventions to improve mobility, strength, balance, gait, and functional independence; adjust treatments as needed.
- Document patient progress, maintain timely records, and communicate updates to the interdisciplinary care team.
- Monitor patient safety, manage activity tolerance, and adapt interventions to meet changing clinical needs.
- Provide education to residents and families; participate in discharge planning and referrals when appropriate.
- Supervise therapy assistants as allowed by state regulations; ensure compliance with facility policies and regulatory requirements.
Requirements
- Active license to practice physical therapy (PT) or eligible status for PT/ PTA in the United States; state-specific requirements apply.
- Ability to work in a long-term care / skilled nursing facility setting.
- Availability for a fixed 11-week travel assignment starting 10/24/2026 with guaranteed hours.
- Strong communication skills and ability to collaborate with an interdisciplinary team.
