Responsibilities
- Perform initial and ongoing physical therapy evaluations to determine patient needs and develop individualized treatment plans.
- Implement evidence-based PT interventions, including therapeutic exercises, gait training, manual therapy, and functional activities to improve mobility and independence.
- Reassess patient progress regularly and adjust treatment plans to meet goals and optimize outcomes.
- Document patient status, progress, and treatment plans accurately in the medical record; communicate findings with the interdisciplinary care team.
- Educate patients and families on home exercise programs, safety, and care planning; promote preventive strategies to reduce fall risk and functional decline.
- Ensure patient safety and adhere to professional standards, facility policies, and regulatory requirements.
Requirements
- Active New York State Physical Therapist license
- BLS certification
- Prior experience in a Skilled Nursing Facility (SNF) required
- Degree in Physical Therapy from an accredited program
- Ability to work in a skilled nursing facility environment
- Availability for a 12-week travel assignment starting 10/26/2026
- Strong communication and collaboration skills with interdisciplinary teams
