Responsibilities
- Provide ER-level nursing care in a home hospice setting, including triage, rapid assessment, and stabilization of acute symptoms.
- Administer medications and treatments per physician orders; monitor patient status and respond to changes in condition.
- Document care and interventions in the electronic health record; collaborate with physicians and the care team to adjust care plans as needed.
- Educate patients and families on symptom management, medications, and equipment use; coordinate with hospice services to support comfort and safety.
- Ensure patient safety and infection control; follow fire and life safety protocols; escalate to on-call or supervising clinicians when appropriate.
- Facilitate smooth transitions of care, including timely handoffs and discharge planning when appropriate.
Requirements
- Current California RN license
- BLS and ACLS certifications required
- LA Hospital Fire and Life Safety card required
- Cerner and Epic experience: preferred; please indicate if you have these EHR systems
- Ability to work in a home healthcare - hospice setting
- Start date availability: 05/11/2026 (as noted in assignment details)
