Responsibilities
- Assess residents to identify PT needs and establish measurable goals
- Develop and implement individualized treatment plans and therapeutic interventions
- Administer therapeutic exercises, balance training, gait training, and manual therapy as appropriate
- Reassess progress, document outcomes, and adjust plans to support function and safety
- Collaborate with nursing and therapy colleagues, participate in discharge planning, and provide education to residents and families
- Ensure accurate and timely documentation in the chart and compliance with organizational policies
Requirements
- Active physical therapy license in Washington or eligible for licensure
- Willingness to participate in a travel assignment and 5x8 day shifts
- Experience in skilled nursing or long-term care setting preferred
- Strong communication, documentation, and team collaboration skills
- Availability to start by 08/31/2026 and travel to the assignment location as required
