The role supports an off-shifts schedule (evenings/nights/weekends/holidays) across multiple units in a SNF. EPIC charting experience is required, and the facility operates with a staff mix that includes physical therapy, interpretation services, phlebotomy, radiology, social services, pharmacy, and unit secretaries. Orientation is 80 hours, with pre-approved time off per posting.

Responsibilities
- Provide charge and supervisory leadership across multiple units, ensuring safe, compliant geriatric nursing care.
- Coordinate care delivery, delegate tasks to nursing staff, and support performance management and staff development.
- Oversee wound care, PICC line management, tube feeds, IV starts/maintenance, and related procedures as part of skilled nursing and rehab services.
- Manage emergencies, participate in code/response activities, and uphold infection control and isolation protocols.
- Monitor regulatory compliance (DPH, TJC, OSHA), incident reporting, and risk management; ensure HIPAA compliance and patient-rights protections.
- Collaborate with PT, pharmacy, social services, radiology, interpretation services, and unit secretaries to optimize patient flow and care continuity; support de-escalation strategies for behavioral challenges.
Requirements
- Active New York RN license in hand to submit
- BLS certification
- EPIC charting experience
- Minimum 2 years RN experience with prior charge/supervisory responsibilities
- Ability to supervise across five units in a 180-bed SNF (neurobehavioral, rehab, secure dementia, long-term care)
- 80-hour orientation and completion of pre-start modules
