Responsibilities
- Assess patients’ healthcare needs and develop comprehensive care plans with an interdisciplinary team, incorporating patient and family goals across biophysical, psychosocial, environmental, functional, and financial factors
- Facilitate clear communication among patients, families, and healthcare providers; coordinate transitions between care settings (hospital to home, rehab, long-term care)
- Evaluate medical necessity for treatments and hospital stays; collaborate with insurance for authorizations; monitor length of stay and identify discharge barriers
- Advocate for patient needs and preferences; educate patients and families about diagnoses, treatments, and self-care; connect with community resources and post-acute services
- Maintain accurate case management records, track outcomes and quality metrics, and prepare reports for care teams and administration; participate in interdisciplinary care conferences
- Coordinate referrals to specialized services (housing, healthcare, legal aid) and consult with specialists as needed; assist with post-discharge planning and continuity of care
- Adhere to required education for applicable program standards (including DNV-related education) and participate in department-wide initiatives; agency staff may be floated to other departments per skill qualification

Requirements
- Licenses/ certifications: Active Oregon RN license; CRASE training required (If travel extends beyond 6 months, CRASE training will be required)
- Certifications: BLS certification preferred; Case Management Certification preferred
- Education: Required: graduate of an accredited nursing program; Preferred: BSN or MSN
- Experience: Required: prior clinical experience in acute care or home health assessing skilled care needs and equipment; Preferred: discharge planning and/or case management experience
- Additional: Bilingual and/or English-Spanish speaking preferred; ability to work with culturally diverse populations
- Physical/working conditions: • Ability to perform essential job functions in an indoor healthcare setting with exposure to infectious conditions; lifts up to 50 pounds with assistance as needed; frequent walking/standing; use of PPE as required
