Responsibilities
- Administer prescribed medications and treatments with accurate verification and documentation
- Monitor residents’ conditions, perform routine assessments, and report changes to the RN or charge nurse
- Implement and update care plans in collaboration with the interdisciplinary team
- Assist with activities of daily living and provide safety-focused resident care during night hours
- Conduct wound care, dressing changes, and routine clinical tasks appropriate to long-term care
- Maintain accurate charting in the electronic health record and document incidents or changes in status
- Participate in rounds, ensure infection control practices, and support falls prevention and safety measures
- Support admissions, discharges, and transitional care activities as part of the care team
Requirements
- Active LPN license in New Mexico
- Basic Life Support (BLS) certification
- Graduation from an approved practical nursing program
- Ability to work 12-hour night shifts and meet 36-hour weekly guarantee
- Experience in a long-term care or skilled nursing setting preferred
- Subject to credentialing, background check, and drug screening as part of onboarding
