A rural healthcare organization is seeking a patient-centered physical therapist to join our team for a travel, temporary assignment. This position provides 40 guaranteed hours per week with a 13-week contract. The shift runs from 8:00 a.m. to 5:00 p.m., and the work setting includes both skilled nursing and hospital environments. The assignment offers a competitive weekly pay range and a stable schedule for the duration of the contract.

Responsibilities
- Perform patient evaluations and develop individualized treatment plans for adults in acute, subacute, or post-acute settings
- Implement evidence-based physical therapy interventions, including therapeutic exercise, gait training, manual therapy, and modalities as appropriate
- Reassess progress and adjust treatment plans to optimize mobility, function, and safety
- Document evaluations, treatment plans, progress notes, and discharge plans in accordance with facility and regulatory requirements
- Collaborate with physicians, nurses, and other members of the interdisciplinary team; participate in rounds and care planning
- Educate patients and families on home exercise programs, safety, and functional goals; promote patient-centered care and informed decision-making

Requirements
- Active physical therapy license with eligibility for licensure in New Mexico (or current NM licensure)
- Graduate of an accredited Physical Therapy program (DPT or relevant advanced degree)
- Experience in hospital and/or skilled nursing facility settings is preferred
- Ability to work in a fast-paced environment and to adjust to patient needs and safety considerations
- Strong communication skills and collaboration with multidisciplinary teams
- Willingness to work the designated shift and travel as required by the assignment
