Responsibilities
- Provide direct nursing care to residents per individualized care plans under RN supervision.
- Administer prescribed medications and document administration; monitor for effectiveness and adverse reactions.
- Monitor resident condition through regular vital signs, changes in condition, and symptom tracking; escalate to supervising RN as needed.
- Support activities of daily living (bath, grooming, dressing, toileting), mobility assistance, and safety measures.
- Perform routine nursing procedures within scope of practice (e.g., wound care, dressing changes, specimen collection) as trained.
- Maintain accurate charting and documentation; ensure privacy, confidentiality, and adherence to infection prevention and safety standards.
- Collaborate with the interprofessional team and communicate changes in residents’ conditions with families or designated contacts when appropriate.
Requirements
- Active Licensed Practical Nurse license in good standing and ability to practice; licensure needs may require state eligibility or transfer.
- 1 year of recent nursing experience in a long-term care or skilled nursing setting preferred.
- Ability to work 12-hour shifts; flexible schedule to support travel assignment needs.
- Strong communication, assessment, and documentation skills; effective teamwork and collaboration.
- Proficiency with basic computer-based documentation and following care plans accurately.
- Willingness to participate in a 12-week travel assignment and relocate as needed.
