Licensed Practical Nurse (LPN) – Long-Term Care travel assignment in Geneva, NY. Start date 05/05/2026 for a 13-week contract. Weekly pay $392-$413. Guaranteed hours: not guaranteed. Shift: Nights on a 2x8 schedule. Setting: Skilled Nursing Facility providing care to adult and geriatric residents. This is a travel - temporary assignment.

Responsibilities
- Collect data on residents’ physical and psychosocial condition on an ongoing basis and communicate findings to the RN and the Interdisciplinary Team.
- Contribute to a developmentally appropriate, culturally relevant plan of care reflecting nursing diagnoses and interventions; involve the resident, family, and healthcare team to achieve outcomes and promote a safe environment.
- Recommend revisions to the plan of care after evaluating resident outcomes and indicate the need for referrals to ensure all aspects of care are addressed; collaborate with resident/family and team within the scope of nursing practice.
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse; maintain current knowledge through continuing education and inservice programs.
- Demonstrate leadership in the professional practice setting; participate in mentoring or orientation of staff and consider safety, cost, and impact in care delivery.
- Adhere to infection control and safety protocols; maintain awareness of exposure to bloodborne pathogens and perform tasks accordingly. Occasional weekend/night coverage may be required, including every other weekend (Fri & Sat nights) as part of the schedule.
Requirements
- Active New York LPN licensure
- BLS certification (AHA or RQI accepted)
- Minimum 1 year of LPN experience in a long-term care setting
- Education: Graduate of an approved practical nursing program; current LPN license
- Ability to work in a skilled nursing facility environment and collaborate with the interdisciplinary team
- Strong assessment, communication, and documentation skills
