Responsibilities
- Perform comprehensive PT evaluations for residents in a skilled nursing facility
- Develop and implement individualized treatment plans aimed at improving mobility and functional independence
- Provide therapy programs including therapeutic exercise, gait training, balance activities, transfers, and activities of daily living training
- Reassess resident progress regularly and update documentation and care plans accordingly
- Collaborate with an interdisciplinary team (nursing, OT, SLP) and communicate with residents and families to support discharge planning and goals
- Ensure compliance with professional standards and facility policies; maintain accurate records and timely charting
Requirements
- Active physical therapy license to practice in the United States (state requirements to be met)
- Ability to work effectively in a skilled nursing facility environment
- Strong assessment, documentation, and communication skills
- Demonstrated ability to collaborate within an interdisciplinary team
- Availability for a 13-week assignment with guaranteed hours
