Responsibilities
- Conduct comprehensive PT evaluations for residents in a long-term care setting; establish measurable goals and develop individualized treatment plans aligned with resident needs.
- Implement, monitor, and adjust evidence-based interventions to improve mobility, balance, strength, gait, and functional independence in daily activities.
- Document assessments, treatment plans, progress notes, and outcomes in the resident’s electronic medical record in accordance with facility policies.
- Collaborate with an interdisciplinary team including nursing, occupational therapy, speech therapy, and physicians to coordinate care, ensure safety, and optimize discharge planning.
- Educate patients and families on home exercise programs, safety strategies, fall prevention, and caregiver training to support continued progress after formal therapy ends.
- Ensure adherence to infection control, safety standards, and professional ethical guidelines; participate in quality improvement activities as applicable.
Requirements
- Active physical therapy license in New Hampshire (or eligibility for NH licensure)
- Ability to work 40 hours per week with a day shift schedule (08:00–16:30)
- Willingness to practice in a long-term care setting and collaborate with a multidisciplinary team
- Strong communication, documentation, and teamwork skills
