Responsibilities
- Provide direct resident care in a long-term care setting under supervision, including observation, assessment, and reporting changes in condition
- Administer medications and treatments per the plan of care, monitor for adverse reactions, and document administrations accurately
- Assist residents with activities of daily living and mobility, promoting safety and independence
- Collect and record vital signs, prepare and maintain essential documentation in the charting system, and communicate pertinent information to the care team
- Collaborate with licensed nursing staff and interdisciplinary team to implement care plans and ensure quality resident outcomes
- Adhere to infection prevention and safety protocols, maintain a clean and safe care environment, and comply with regulatory requirements
Requirements
- Active LPN license in good standing
- Current CPR/BLS certification
- Ability to work evening and/or overnight shifts
- Comfortable delivering care in a long-term care setting
- Strong communication, documentation, and teamwork skills
- US work authorization
