Responsibilities
- Provide direct patient care for rehabilitation patients, including initial assessment, ongoing observation, and documentation of progress toward functional goals in accordance with facility policies.
- Administer medications and treatments as prescribed; monitor effects, identify adverse reactions, and escalate concerns promptly.
- Collaborate with physical therapy, occupational therapy, speech therapy, and wound care teams to implement comprehensive rehab plans and adjust goals as patients progress.
- Monitor vital signs, manage pain, assess fall risk, and maintain accurate nursing notes in the electronic health record; communicate changes during shift handoffs.
- Support activities of daily living, mobility training, and safety measures during night shifts; promote patient comfort and dignity.
- Ensure compliance with nursing standards, infection prevention, and patient privacy; participate in care coordination and discharge planning.

Requirements
- Active Registered Nurse license; state-specific licensure requirements apply.
- Basic Life Support (BLS) certification.
- Rehab or skilled nursing experience preferred.
- Ability to work a 3x12 night shift and adapt to a night-oriented schedule.
- Strong documentation and interpersonal skills; ability to collaborate with an interdisciplinary team.
- Eligible for a 13-week travel assignment in a skilled nursing setting.
