Licensed Practical Nurse (LPN) – Long Term Care, Bala Cynwyd, PA, starting 08/26/2026 for 12 weeks. Weekly pay range: $1,270–$1,317 per week with guaranteed 40 hours. Shift is 5x8 days (8:00 AM–4:30 PM) in a home healthcare setting, under a Local Contract (Nursing) program.

Responsibilities
- Deliver skilled LPN nursing care during home visits, with a focus on wound care management and documentation.
- Assess patient condition, monitor vital signs, administer prescribed medications, and implement plan of care under supervision.
- Perform wound care procedures, dressing changes, and routine clinical tasks in accordance with clinical policies.
- Maintain accurate and timely charting in the electronic medical record; communicate updates to the RN supervisor and care team.
- Educate patients and families on care plans, medication administration, safety, and self-management strategies.
- Coordinate with physicians, therapists, and other providers to ensure continuity of care and adherence to care plans.
Requirements
- Active Licensed Practical Nurse (LPN) license in Pennsylvania (or eligible for licensure in PA)
- Minimum 2 years of home care experience
- Proficiency in wound care required
- Ability to work in a home healthcare setting and cover patient visits in 19104 and 19139 zip codes
- Comply with agency policies, documentation standards, and patient safety guidelines
