Responsibilities
- Provide direct patient care to adult residents in a long-term care and acute rehab setting, under supervision of an RN
- Administer medications and perform treatments within scope of practice and facility policy
- Monitor patient status, vital signs, and response to therapies; document and communicate any changes promptly
- Assist with activities of daily living and support rehabilitation activities as directed
- Collaborate with the interdisciplinary team to implement and adjust care plans
- Maintain accurate documentation, uphold safety standards, and comply with regulatory requirements
Requirements
- Active North Carolina LPN license or a compact license
- Basic Life Support (BLS) certification
- 1+ year of geriatric LPN experience
- Ability to work evening shifts in a LTC/acute rehab setting
- Willingness to travel for a 13-week assignment
- Strong communication and documentation skills
