Responsibilities
- Perform comprehensive speech-language assessments for residents, focusing on communication and swallowing needs in a SNF setting.
- Develop and implement individualized treatment plans; provide direct therapy and monitor progress toward goals.
- Document therapy assessments, treatment sessions, progress notes, and outcomes in Rehab Optima (Net Health) and other required systems; ensure accuracy for billing and compliance.
- Collaborate with interdisciplinary team (nursing, OT, PT, dietary, social work) to support rehabilitation goals and discharge planning.
- Maintain compliance with state licensure requirements and facility policies; ensure patient safety, confidentiality, and ethical practice.
- Educate residents and families on communication strategies and swallowing safety; provide staff training as needed.
Requirements
- Active California Speech-Language Pathologist license
- More than 1 year of experience in a skilled nursing facility environment
- Knowledge of Rehab Optima (Net Health) or demonstrated proficiency with Rehab Optima
- Ability to work a daytime schedule
- Eligible for a 13-week travel assignment
