Responsibilities
- Conduct initial assessments and ongoing PT evaluations for residents in a skilled nursing setting.
- Develop and implement individualized treatment plans focusing on functional mobility, gait, balance, strength, ROM, and activities of daily living.
- Provide therapeutic interventions such as therapeutic exercise, gait training, manual therapy, and pain management as appropriate.
- Monitor patient progress, document treatment notes, and adjust plans of care in collaboration with an interdisciplinary team.
- Educate patients and families on exercises, safety strategies, and home programs; ensure safe transfers and fall-prevention practices.
- Ensure compliance with facility policies, state regulations, and documentation standards; maintain patient safety and privacy at all times.
Requirements
- Active Physical Therapist license to practice in California.
- Graduation from an accredited physical therapy program (DPT preferred).
- Experience in skilled nursing facility or geriatric rehabilitation setting preferred.
- Ability to work effectively within an interdisciplinary team in a SNF environment.
- Basic life support (BLS) certification or equivalent as applicable.
- Willingness to travel for a temporary assignment.
