Responsibilities
- Administer prescribed medications and treatments per physician orders and facility policy
- Monitor patient status, vital signs, and response to interventions; document findings accurately

- Collaborate with RNs and other staff to implement and update care plans
- Ensure safety, infection control, and accurate charting in the electronic medical record
- Communicate effectively with patients, families, and team members; escalate concerns as needed

Requirements
- Active LPN license in Pennsylvania (or eligible for licensure in state)
- Diploma from an accredited practical nursing program
- Prior long-term care or skilled nursing experience preferred
- Ability to work in a fast-paced, team-based long-term care environment
- Strong documentation and communication skills
- Eligibility to participate in a travel assignment in Pennsylvania
