Responsibilities
- Conduct initial and ongoing occupational therapy assessments for adults in a skilled nursing setting; develop and implement individualized treatment plans aimed at improving activities of daily living, mobility, cognition, balance, and safety.
- Deliver evidence-based OT interventions; tailor activities to patient goals; monitor progress and adjust treatment plans as needed; document progress and discharge planning.
- Collaborate with an interdisciplinary team (nursing, therapy colleagues, social work) to coordinate care, plan transitions, and optimize patient outcomes.
- Maintain accurate, timely documentation of evaluations, treatment sessions, progress notes, and discharge summaries in accordance with regulatory and facility requirements.
- Educate patients and families on home exercise programs, safety strategies, and environmental adaptations to support independence and reduce risk of decline.
- Ensure compliance with facility policies, state and federal regulations, and patient privacy standards.
Requirements
- Active Occupational Therapist license (state-specific) or eligibility to practice in New Mexico.
- Experience in skilled nursing facility or geriatric rehabilitation preferred.
- Ability to work effectively in a skilled nursing setting and to collaborate with an interdisciplinary team.
- Strong documentation, communication, and organizational skills.
- Travel assignment readiness and flexibility to work the outlined shift and schedule.
