Responsibilities
- Perform comprehensive discharge planning and care coordination for a caseload of approximately 25–30 patients across multiple inpatient units.
- Assess post-acute needs, arrange appropriate services, DME, and social work support; develop and monitor the plan of care with an emphasis on safe, timely transitions.
- Collaborate with interdisciplinary colleagues (including Social Services) to facilitate appropriate placements and resource utilization.
- Document all assessments, plans, and patient progress in EPIC, ensuring timely communication of changes to the care team.
- Provide cross-coverage across hospital areas and float to similar units as needed; operate with a patient-age range from Neonates to Geriatrics.
- Adhere to unit policies, maintain compliance with discharge criteria, and participate in pre-start training and orientation requirements.

Requirements
- Active RN license in Washington (or eligibility; pending license accepted per posting) and BLS certificate.
- Minimum 1 year of acute care experience with discharge planning / care coordination; strong background in diverse case management preferred.
- Experience managing a 25–30 patient caseload; proficient in needs assessment, POC development, and care referrals; EPIC charting experience required.
- Ability to work in an acute care setting and float across units and to a second campus within close proximity; comfortable caring for patients across the age spectrum (Neonates–Geriatrics).
- Travel assignment; local candidates not accepted; must be able to meet pre-start and orientation requirements; RTO (time off) must be listed at submittal (up to 5 days typically allowed); requires two supervisory references covering the last year of employment.
