Responsibilities
- Conduct comprehensive physical therapy evaluations and develop individualized treatment plans for residents within the facility.
- Implement evidence-based therapy interventions to improve mobility, strength, balance, gait, and functional independence.
- Reassess patient progress regularly, adjust goals, and document outcomes in accordance with facility policies and regulatory requirements.
- Maintain accurate, timely documentation and collaborate with the interdisciplinary care team to coordinate resident care.
- Educate residents and families on home exercise programs, safety strategies, and fall prevention.
Note: Agencies must have NM caregivers account increased rate.
Requirements
- Ability to provide physical therapy services in a skilled nursing facility setting.
- Strong clinical assessment, treatment planning, and documentation skills.
- Effective communication and teamwork abilities to coordinate with nurses, physicians, and other therapists.
- Willingness to work on-site in Silver City, NM and adhere to the assigned schedule.
