- Collaborate with a multidisciplinary team to implement and adjust individualized care plans
- Ensure a safe, clean, and respectful environment; respond promptly to calls and deteriorations
Requirements
- Active LPN license or eligibility for licensure in New Hampshire
- Prior experience in long-term care or skilled nursing settings preferred
- Ability to work night shift and meet guaranteed hours
- Strong communication and documentation skills
- CPR/BLS certification preferred
- Ability to follow care plans, perform procedures, and adhere to regulatory requirements
