Responsibilities
- Plan and conduct segments of PT treatment programs to restore and improve physical, social, and mental functions, aligned with department objectives.
- Co-lead interdisciplinary patient groups and coordinate care with other team members.
- Complete comprehensive evaluations, establish individualized treatment plans, and implement evidence-based interventions.
- Develop and communicate care plans, including discharge planning, to patients, families, and care team.
- Document clinical findings and progress in the patient record per established timelines, using the facility’s electronic system (EPIC experience preferred).
- Supervise assigned assistants and contribute to program quality through accurate clinical documentation and teamwork.
Requirements
- Education: Doctor of Physical Therapy (DPT) degree required; only if obtained license on or after January 2016; or foreign equivalent.
- Licenses/ Certifications: Physical Therapy licensure in state of practice; CPR/BLS required (AHA-BLS or AHA-CPR); Valid Driver’s License; must be at least 21 years old.
- Experience: At least two years of recent experience in the field.
- Knowledge/Skills: Must know HCHB and OASIS; EPIC experience preferred; 1 year of home health experience; ability to work in a hospital setting and function within an interdisciplinary team.
- Licensing Notes: Licensure in all applicable states of practice; current nursing license per the state of assignment.
