This is a travel, temporary assignment for a Physical Therapist providing outpatient services in a rural, critical access setting. The assignment runs for 13 weeks with a standard 5-day workweek and a daytime schedule.

Responsibilities
- Conduct initial and ongoing PT evaluations for outpatient patients.
- Develop, implement, and monitor individualized plans of care with measurable goals.
- Provide therapeutic exercises, gait and balance training, and functional mobility interventions.
- Apply evidence-based modalities and, when appropriate, manual therapy techniques.
- Reassess patient progress regularly; adjust plans and document changes in the electronic health record.
- Educate patients and families on home exercise programs, safety, and post-discharge plans.
- Collaborate with the care team (physicians, nurses, case managers) to coordinate care and ensure smooth transitions.
Requirements
- Education: Bachelor’s Degree in Physical Therapy from an accredited program, or a degree in a related field.
- Licenses/Certifications: California Physical Therapy License (active); AHA BLS Certification.
- Experience: Prior physical therapy experience preferred; outpatient/home health experience preferred.
- Other: Ability to work effectively in an outpatient/home health setting within a rural, critical access environment.
- Professional alignment: APTA membership is preferred.
