Responsibilities
- Conduct comprehensive PT evaluations and develop individualized treatment plans for patients in the acute care setting.
- Implement and adjust therapeutic interventions to improve mobility, functional independence, and discharge readiness.
- Reassess patient progress regularly; document outcomes and plan of care in the EPIC system.
- Collaborate with the interdisciplinary team to coordinate care, set goals, and support patient safety.
- Provide patient and family education on home exercise programs, activity modification, and discharge planning.
- Apply appropriate therapeutic modalities and manual techniques within scope of practice and facility policies.
Requirements
- Current nursing license issued by the state of assignment
- At least two years of recent experience in the field
- Current BLS certification
- Epic EMR experience
- Ability to work in an acute care hospital setting
