Responsibilities
- Provide direct nursing care for sub-acute/long-stay rehabilitation patients in a hospital setting, focusing on medical management and functional recovery
- Assess patient conditions, develop and implement care plans, monitor vitals, and administer medications per physician orders
- Chart and document in EPIC EMR; collaborate with multidisciplinary rehab team to coordinate therapies, goals, and discharge planning
- Monitor for neurological changes, apply NIH Stroke Scale as required, and respond to emergencies per BLS/ACLS guidelines
- Support rehabilitation goals through close coordination with nursing, therapy, and ancillary staff; ensure patient safety and infection control
- Adhere to all facility policies, maintain accurate records, and participate in scheduled orientations and required trainings

Requirements
- Current RN license in Vermont or eligibility for licensure in Vermont
- BLS and ACLS certifications; NIH Stroke Scale competency/verification prior to starting
- EPIC EMR experience within the past 2 years
- Rehab/LTC experience required
- Ability to work in a hospital setting; 12-hour night shifts; no float to other units
- Reliable personal transportation to and from assignment; area has limited public transit; winter driving with appropriate tires recommended
