Responsibilities

- Assist residents with activities of daily living and mobility to promote comfort and independence
- Maintain accurate documentation in the charting system, including assessments, care plans, and progress notes
- Collaborate with the interdisciplinary team to support care planning, safety, and discharge planning as needed
Requirements
- Active LPN license in good standing (state of assignment)
- Graduation from an accredited practical nursing program (diploma)
- Ability to work in a long-term care setting and adapt to evolving resident needs
- Strong communication and teamwork skills, with a focus on resident-centered care
- Prior experience in long-term care or geriatric nursing preferred
- Basic understanding of nursing procedures and documentation standards
