Responsibilities
- Evaluate, plan, direct, and administer Physical Therapy services as prescribed by a licensed Provider in inpatient, outpatient, and/or home health settings.
- Treat patients using PT modalities (e.g., ultrasound, NMES, iontophoresis) and skilled techniques; instruct patients on the use of adaptive/assistive equipment.
- Supervise and direct Physical Therapy Assistants; maintain open communication with the Rehab team and consult other departments as needed.
- Perform appropriate assessments on all patients and interpret results within licensure scope; design treatment programs and set realistic short- and long-term goals with patient and family involvement.
- Monitor treatment effects, adjust plans as necessary, ensure accurate and timely documentation and charges, and participate in performance improvement activities.
- Ensure patient safety and compliance with regulatory standards; maintain thorough documentation, including evaluations, progress notes, monthly notes and discharge summaries.

Requirements
- Current IL Physical Therapist license required
- BLS certification required
- Valid driver's license and proof of personal auto insurance required
- Physical Therapy experience: minimum 1 year preferred
- Ability to work in inpatient, outpatient, and/or home health settings; strong communication and teamwork skills
- APTA membership preferred
- Proficient computer skills (Word, Excel, Outlook)
